## Bias Education and Response

Welcome to the University’s Bias Education and Response System. Please find below a form to report instances of bias related to protected classes (race, gender, age, disability, sexual orientation, etc.) against another member of the community.

This form allows you to identify the type of response desired by the University’s Bias Education and Response Team, which consists of trained faculty members, administrators, and students. You may select from a variety of informal resolutions, including education, training, and/or mediation. This process is meant to be **educational not punitive**; therefore, if you are seeking an investigation and accountability into an alleged act of discrimination, please consult the University’s site for reporting acts of discrimination. This form below does allow for anonymous reporting. However, please understand that if you choose not to identify yourself, the team’s options are limited in their attempt to resolve your complaint.
