## Health Services Information

### Mandatory Forms & Required Action

Manhattan University requires all students registered for 6 or more credits on campus at Manhattan University to comply with New York State law regarding college immunizations.

**Health Records:** Download the required immunization and physical forms by registering at [medproctor.com](https://www.medproctor.com/). Forms are due by August 1. For more information, please visit [manhattan.edu/HealthRecords](/HealthRecords).

**Health Insurance:** Beginning June 2026, complete a waiver with evidence of health insurance or enroll in the University Student Health Insurance Plan. Enroll or waiver is due August 1. For more information, please visit [manhattan.edu/StudentHealthInsurance](/StudentHealthInsurance). International F1 Visa students who meet the criteria are automatically enrolled into the MU-SHIP insurance.
